Use and complete the DOWNLOADABLE form linked below and then you can hand delivery or mail your application to the following address

You must fill in all required sections as noted on this application prior to returning to the Clerk's Office
On receipt it can take up to 3 business days for a Deputy Clerk to review your application and reach out with further instructions.
